Obverse: half-figure of Sigismund III Vasa facing right. Shoulder with two rows of spirals with large decorative rosettes between them. Wing with crosses. A six-barrel crown with inner bails that do not descend in the globe. Four large rosettes are placed in the space between the bail. Apple with two jewel bands, horizontal curved downwards. Both hemispheres without decoration. The king holds a broad sword with one shavings in his hand. Linearized scarves. The sash, built of thin lines, is placed in a mirror image to the normal position. The chain of the Order of the Golden Fleece contains flints, between which are placed vertical sticks instead of flint. Both of the apple jewel belts contain crosses in the normal position. The sparsely spaced spirals forming the decoration of the breastplate are present in the second position. Spirals placed in the epaulette are visible in the same position. The bow with two ribbons ending in short tassels and one large knot. The image of the king is devoid of the hand holding the royal apple., in the outer border the legend:

Reverse: A five-field shield with the coats of arms of the Crown, the Grand Duchy of Lithuania and the coat of arms of the Vasa family, crowned with a crown with a closed rim with 6 cross-shaped jewels, with two fleurons, with a reticulated background, surmounted by a royal apple; On the sides of the shield the date 16-22 and labrys in the shape of snails, in a corded rim, in the outer rim a legend punctuated by the coat of arms of Sas Treasurer Mikolaj Danilowicz in an ornamental shield:
