Obverse: half-figure of Sigismund III Vasa facing right. The epaulet with two rows of small circles with rosettes between them. The wing contains only rosettes. Four-bar crown without circles and crosses on fleurons. Apple without ornament with two jewel bands. The king holds a broad sword with visible shavings. Pollinated sash. The surface of the sash contains two intertwined lines with edges made of circles., in the outer border the legend:

Reverse: A five-field escutcheon with the coats of arms of the Crown, the Grand Duchy of Lithuania and the coat of arms of the Vasa family, crowned with a crown with a closed rim with 6 cross-shaped jewels, with two fleurons, with a reticulated background, topped with a royal apple; On the sides of the shield the date 16-21 and labrys in the shape of snails, in a corded rim, in the outer rim a legend punctuated by the coat of arms of Sas Treasurer Mikolaj Danilowicz in an ornamental shield:
