Obverse: half-figure of Sigismund III Vasa facing right. epaulet with two rows of small circles withrosettes between them . The wing contains only rosettes. Thefour-bar crown without a cap, with one row of jewels and three rosettes. Two smaller rosettes are placed on both fleurons, the third - triangular and larger above the heraldic lily. An apple without decoration with two bands of jewels. The king holds a narrow sword with a tapering head with two shavings. The sash is linear. The sash is formed by three parallel lines on an openwork base. The larger rosette in the crown completely covers the globe. The bow with two joined ribbons, without a knot. The chain of the Order of the Golden Fleece contains flints made of two bails, the longer ends of which are braided and wrapped inward. The flint, which together with the chair forms the link of the chain, contains a large oval gemstone from which two bundles of rays emerge. Two rosettes are visible in the apple's vertical jewel band, with a legend in the outer rim:

Reverse: five-field escutcheon with the coats of arms of the Crown, the Grand Duchy of Lithuania and the coat of arms of the Vasa family, crowned with a crown with a closed rim with numerous jewels, without fleurons, with a reticulated background, surmounted by a royal apple; On the sides of the escutcheon, the date 16-21 and labrys in the shape of snails, in a rope border, in the outer rim the legend punctuated by the coat of arms of Sas Treasurer Mikolaj Danilowicz in an ornamental shield:
